Alliance Homecare is looking to hiring a hard-working Intake and Supervision RN to coordinate LTC assessments and oversee the quality care provided by home health aides.
Required Skills/Experience: Registered Nurse in New York with a clean license
Bachelors degree from a four-year accredited university preferred, Associates Degree requires Minimum of three years of experience in a healthcare setting, preferably in a clinical care arena
Outstanding decision-making and problem-solving skills
Technologically savvy, and proficient with Microsoft Office Suite and Google office tools
Thrive working in a fast-paced environment
Must be a self-starter with demonstrated initiative and ability to multi-task
Excellent verbal and written communication skills
Energetic, friendly and organized
Willing to be field-based and travel around the five boroughs of New York and western Long Island Key Responsibilities: Conduct quarterly supervisory home visits to review the Care Plan with caregiver and client, to assure home health aides are properly following the Plan of Care, and provide opportunity for the HHA to communicate any concerns
Ensure clients needs are being met during quarterly visits by conducting a physical assessment of the client and to engage in a dialogue to address any concerns, and compose a 487 for any changes in medication or care
Complete accurate documentation following the supervision visit
Engage in dialogue with care team members regarding the caregivers ability to follow care plan and possible need for training
Conduct LTC Assessments providing by Company providers by reviewing client rights and responsibilities, obtain any necessary signatures and accurately complete the comprehensive questionnaire
Follow all documentation and communication protocol as defined by Company providers
Assist in promoting Company services during LTC assessments to drive new business
Keep abreast of industry trends and regulatory requirements through activities such as attending conferences and reviewing industry publications
Assures to delivery of quality homecare by collecting, analyzing, trending and monitoring data and incorporating the data in the quality improvement plan
